GOOD FAITH ESTIMATE NOTICE
Under California law, you have the right to receive a “Good Faith Estimate” to give you insight on the estimated cost of care working with health practitioners. Here are some things for you to know surrounding this law:

-
By law, I have to provide you the expected charge for the medical services I provide when you are not using insurance or do not have it.
-
You can ask me, your dietitian, and any other provider you choose, for a Good Faith Estimate before you select a service to work with me.

For more information on the Good Faith Estimate act to avoid surprise bills: visit www.cms.gov/nosurprises or call 1-800-985-3059.
